Cat Adoption Agreement

I, _______________________________________________ hereby acknowledge adopting the above described cat/kitten from Goliath & Be-Be’s World and agree to:

1.       Provide this animal with proper humane care for the duration of its entire natural life; including, but not limited to, food, fresh water, shelter, affection, exercise and all other necessities; and will keep this animal as an indoor only pet.

2.       Provide routine veterinary care to include at a minimum, an annual health examination and routine vaccinations.

3.       I will not give away, sell, trade, lend or lease this animal; I will notify Goliath & Be-Be’s World immediately if I find, for any reason, that I am unable to keep this animal. I agree to continue to care for the animal in my home until Goliath & Be-Be’s World can move the animal into a foster home or permanent home. I will never have the animal euthanized for any reason other than an end-stage injury or illness where the animal is suffering, and in that case, the euthanasia must be performed by a licensed veterinarian under supervised conditions.

4.       I will not surrender this animal to any animal shelter that euthanizes surplus pets, or to any third party without the express approval of Goliath & Be-Be’s World, or release this animal to fend for him/herself.

5.       I agree to make a serious effort to find the cat/kitten if it becomes lost, by immediately (a) conducting a detailed physical search of my yard and neighboring yards, (b) setting humane traps in my yard to catch the cat, (c) filing lost reports with the local police, animal control authorities, animal shelters, SPCA's, and local veterinarians, (d) posting lost cat signs, and (e) contacting us for additional advice.

6.       I acknowledge that Goliath & Be-Be’s World has had this animal for a limited time. Goliath & Be-Be’s World makes no representation that the animal is free from characteristics or propensities that could be a source of danger to humans, other animals or to property. _______(initials required).

7.       I also acknowledge that, while Goliath & Be-Be’s World takes every precaution to assure the health of its cats/kittens, no guarantee of any kind can be given regarding the condition of the animal I have adopted. I understand that any medical bills incurred will be solely my responsibility. _______(initials required).

8.       I understand that all information provided on the written application and during the personal interview is true and correct to the best of my knowledge, and that any false or misleading information will nullify this contract and require the immediate return of the cat/kitten to Goliath & Be-Be’s World.

9.       I agree to pay attorney’s fees and costs for any litigation or mediation required to enforce this contract or caused by breach of this contract.

I understand and agree to all provisions in this agreement. I accept possession of this pet “as is” and at my own risk. I hereby release and waive any claim against Goliath & Be-Be’s World which I may have now or in the future for any damages and/or injuries to person or property caused by said animal. Abuse, neglect or failure to comply with any of the provisions of this agreement will constitute a breach of contract and will result in the removal of the animal from my custody. Florida law and its applicable statutes, if any, will apply to this contract, including its interpretation and enforcement.
