(approx. 8 weeks of age) and animals going to Animal Home
Health must be over 3 pounds (approx. 12 weeks of age).
Do not feed your animal after 8:00pm the night before
surgery water is fine and necessary. Animals should
be well hydrated before surgery.
You MUST present your certificate when you drop
off your animal. If you are receiving SNIP! assistance,
you must obtain a certificate for each animal.
Please Note: If your pet is pregnant or in heat at the
time of service, the veterinarian may charge an
additional fee. In addition, some clinics may require a
wellness exam and/or pain medication following surgery,
which would be your financial responsibility, as well as
any additional services rendered at the time of your
veterinary appointment.
Please discuss optional services with the clinic of your
choice.
The spaying and neutering of an animal is generally a
safe, routine, surgical procedure; however any surgical
procedure involves risk.
Your signature on the assistance certificate acts as a
release of liability in the unlikely event complications
arise..
Certificates MUST be signed by bearer in order to be
valid. Thank you. Goliath & BeBe's World, Inc.
